• Date of birth
• City and Country of residence 
• Email address
• City and country of purchase (ideally name of retailer)
• Self purchase or gift
• Photo of the front and back of your DENIZEN product

Unless purchased online at our website or a gift, we recommend you save your proof of purchase.
